People generally dont care about the official warnings, since they dont do anything, really.

Generally yes, 3 official warnings is a ban. However it's not always that case.
For people that have been showing signs of being a troll of troublemaker, I usually skip PM warnings and go for the official warning first. Sometimes, and usually for harassment cases, I ban people for 2 warnings.
Continuing after being temporarily bans does not get you a permaban, usually another temporary one which is just longer. Again it depends on the offenses, and moderator's call.

If we're going to go down that path, it should be made inaccessible to those that aren't judges or those with a position of power like the hidden place canned threads go.

And to my knowledge reporting in-game does include lobby chat/name selection chat. That's what I was told, at least.
